Physical Activity Readiness Questionnaire

Prior to your first session you will be asked to fill out a Physical Activity Readiness Questionnaire (PAR-Q). This is to determine any medical conditions that I may need to take into consideration during training, and that you may need to speak to your GP about. Most people can begin exercise without worry. However, some people may require a preliminary health check to ensure that they are taking on the right level of physical activity for their medical condition.

My Commitments

  • I will respect your decision on the exercises you carry out. However, I ask you not to exercise beyond your abilities.

  • I will make sure that my equipment is in safe condition to use.

  • I will make sure that I have the relevant qualifications.

  • If you have special needs, I will consider what adjustments, if any, are reasonable for us to make.

Your Commitments

  • You should not exercise beyond your own abilities. If you have a medical condition which might interfere with your training safely, you should seek advice from a relevant medical professional and follow that advice before you do any training or use any equipment.

  • If there is any equipment which you are unsure how to use, you should ask a qualified trainer.

  • You should not carry out any activity which you have been told is not suitable for you.

  • You should let your trainer know immediately if you feel ill during your session.

  • If you have special needs, you must follow any reasonable instructions to exercise safely.
